PURCHASE AGREEMENT/HEALTH GUARANTEE
This puppy has a ONE (1) Year Replacement Guarantee against any Genetic/Congenital Disease that
causes said puppy to expire. Seller guarantees that to the best of her knowledge this puppy is in
good, sound health, free from symptoms of communicable diseases & is current on vaccinations &
wormings. No further guarantee is made or implied.
1. Buyer must have the puppy examined by a licensed veterinarian within 48 hours of
possession. During the course of the 1 Yr. Guarantee the puppy must be kept up to date on
vaccinations, wormings & any medical care recommended by the Buyers’ vet. The Buyer must be
able to provide records of such care, written & signed by a licensed veterinarian. Failure to comply
voids this guarantee.
2. In the unlikely case a death should occur, an Autopsy Report, with findings stating that the
puppy expired from a genetic/congenital related disorder, along with the puppy's medical records,
must be submitted, written & signed by a licensed veterinarian, to The Breeder. If the breeder’s
vet confirms the diagnosis, the Breeder will give a replacement puppy of the same sex and of equal
value the next time one is available. In the case of a discrepancy between a genetic and an
acquired condition the Breeder has the sole and absolute final decision.
3. The Breeder will not be responsible for any part of bills, medical or otherwise,
incurred by the Buyer on behalf of the puppy, including transportation charges & those incurred for
a replacement puppy. Refusal of a replacement puppy by buyer ends all obligations Breeder has to
buyer. Should any of the above stated conditions not be followed and/or there is a lack of proper
testing necessary to completely & properly diagnose the puppy, this guarantee is null & void.
4. Conditions considered normal to the Shih Tzu breed such as but not limited to: cherry eye,
stenotic nares, under bite, overbite & both inguinal & umbilical hernias are not covered by this
guarantee. This guarantee does not cover any contagious diseases, internal or external parasites,
viral or bacterial infections, conditions brought on by stress and/or environmental changes, injuries
or losses due to shipping, or any other circumstances that the Breeder cannot control once the
puppy leaves their care. All puppies are sold as companion animals only & are not guaranteed for
show or breeding purposes. NO guarantee is made as to size, color, disposition/personality and/or
life-span.
5. Buyer and seller acknowledge that no representations, verbal or implied, have been
made regarding the puppy. Buyer further acknowledges that this contract will stand in a court of
law & is the final standing agreement between Buyer & Breeder, superseding any other agreements,
verbal or otherwise, made regarding this puppy. Any disputes will be litigated in Delta County,
Colorado at the buyers expense. This Guarantee applies to the original buyer only & is non-
transferable.
In signing this contract, the Buyer verifies that they have read, understand & agree to, all terms
& conditions stated herein, is buying this puppy as is & will assume all responsibilities from this
date on.

Below is a copy of our Health Guarantee & Deposit Agreement,

A $200 non-refundable deposit is required to hold the puppy described below until it is 8 weeks
age, with the balance of the purchase price paid on or before that date.
1.) If the puppy is being shipped, a signed, dated copy of the Health Guarantee must be
submitted before the puppy can be shipped.
2.) If the puppy is to be picked up in person, the Puppy must be picked up when it is 8 weeks old
unless prior arrangements have been made or the puppy is not mature enough to go at that time.
(The decision as to when the puppy is physically mature enough to go to it’s new home is at the
sole discretion of the breeder.)
3.) If the pick up date agreed upon must be rescheduled on the buyer’s behalf for more than 2
weeks past the date agreed upon & prior arrangements have not been made with me, the buyer will
be charged a $20 per week fee for boarding the puppy. If date must be moved due to
weather/driving conditions, the puppy not being physically ready to go or any other reasons
beyond your control, no boarding fee will be charged. In either case, the puppy will continue to
receive care & appropriate shots for his or her age until you pick them up.
4.) Buyer and Breeder acknowledge that no representations, verbal or implied, have been made
regarding the puppy. Buyer further acknowledges that this contract will stand in a court of law.
Any disputes will be litigated in Delta County, Colorado at the buyers expense.
In placing a deposit on a puppy, I verify that I have read, understand & agree to the terms &
conditions stated herein & in the Breeders’ 1 Yr. Health Guarantee.
